Utah Wastewater Operator Certification Program Applications and Forms

This page provides access to the Wastewater Operator Certification Application forms. For a simple renewal, use the Renewal Form. All other requests require the longer form.

Note: A Citizenship/Alien ID Certification Form is required to be in your file as of July 1, 2009. If you have not already submitted one to DWQ, please include it with your application.

Exams, Change of Status, and Reciprocity

Once you have Adobe Reader installed, simply click on the link below to access the application. Don't forget the due dates for exam submittals. Instructions and related fees are listed on Page 4 of the application.  Please include the application with the fees. Good luck on the exam!

    This includes the voluntary certification exams for Wastewater Laboratory Analyst (I-IV), Biosolids Land Applier (I-II), and Plant Maintenance Technologist (I-III). Fill in before printing. Cash, check, money order, or credit card payments are now accepted. A link to the DWQ Shopping Cart is at the top of the application form. You MUST submit an application form as well as the payment.

Reinstatement of Expired Certificates

Recent changes to R317-10 now allow for reinstatement of an expired certificate up to one year after its expiration if the required CEUs were earned prior to its expiration. To reinstate a certificate, use the Renewal form found below unless you also want to Change the Status of a "Restricted" certificate to "unrestricted." Then, use the above form for Exams, Change of Status, and Reciprocity and include all necessary information.

Citizenship/Alien ID Certification

Utah Department of Environmental Quality Certification Pursuant to UCA 63G-12-104 is required for all individuals receiving a certification from the State as of July 1, 2009.

Wastewater Operator Certification Fees

Certification fees may be paid by cash, check, money order, or on-line using the Water Quality Shopping Cart at https://secure.utah.gov/cart/dwq_cart/products.html. On-line payments CANNOT be processed without a corresponding application, so the application form must also be submitted and include the "Order Number" when an on-line payment is used.
