All community and nontransient-noncommunity systems that add a chemical disinfectant to their water must abide by new rules which became effective January 1, 2004.

EPA Quick Reference Guide for Stage 2 Guidance
IDSE (Initial Distribution System Evaluation) forms are required under the disinfection byproducts Phase 2 rule. The four options of the IDSE are:

  • 40/30 Certification
  • Standard Monitoring Form
  • System Specific Study (form not included)
  • Very Small System Waiver

Choose the correct form according to your system water type, ground water or surface water, and population.
