5 Do you or your facility wish to apply to use pre-packaged kits for in-vitro testing distributed under the terms of a General License?


6 Does your facility wish to terminate an existing Specific or General Radioactive Material License?


7 Does your facility wish to make payment for a new, renewed, amended, or annual licensing fee?
