Industry Breakdown Report: Stationary Source Compliance

Industrial facilities must report equipment breakdowns that may cause excess air emissions to the Utah Division of Air Quality. Use the Industry Breakdown Report form to document the nature of the breakdown, pollutants emitted, duration, and corrective actions taken.

Required info

• Company name and location (city and county)
• Contact person and phone number
• Date, time, and duration of the breakdown
• Equipment affected, pollutants emitted, and any excess opacity
• Production levels before and during the breakdown, and actions taken to correct the problem
