  1. All applicants must submit a notarized Citizenship/Alien ID Certification Form (298 KB) (This is a one-time only requirement).
  2. Fill out the application (applications below), attached supporting documents and mail to the address on the application or email to ustcertprogram@utah.gov.

UST testing is offered the first Tuesday of each month at 9:00 AM and the third Tuesday of each month at 2:00 PM. You must register 5 days prior to the exam date.

Petroleum Storage Tank Fees

Annual UST Fee

Provide the Facility ID and invoice number in the additional information box and pay the amount indicated on the invoice that we sent to you.

UST Installation Company Annual Fee

Provide the invoice number in the additional information box and pay the amount indicated on the invoice that we sent to you.

Alternative Mechanism Annual Fee

Provide the invoice number in the additional information box and pay the amount indicated on the invoice that we sent to you.

UST Installation Permit Fees

To pay fees associated with permitting underground storage tank (UST) installations, please provide the facility name, address, DERR facility ID (if available), and permit number (if available) in the “Additional Information” box below. Your DERR project manager can provide the exact amount of the permit fee.

AST Notification Fee

One time notification fee for aboveground petroleum storage tanks. Include your Facility ID # and invoice # in the additional information box.

Decontamination Specialist Certification

A program where an individual can obtain a Certificate to assess and clean up methamphetamine contaminated properties

  1. Fill out the initial or renewal application(s) (applications below) and email with supporting documents, including Proof of Citizenship Form, OSHA HAZWOPER Certificate and payment receipt to kanderson@utah.gov.

Decontamination Specialist Certification–Initial Exam

Decontamination Specialist Certification–Retest Exam

An Applicant is allowed by rule to re-take the initial or renewal exam within one month of the original test date, without submitting a new application, if the Applicant fails the first exam. Prior to the exam, pay the fee and email payment receipt to kanderson@utah.gov.

Enforceable Written Assurance

A letter issued by the DEQ to a prospective purchaser meeting the requirements of the federal bona fide prospective purchaser liability defense and state criteria.

Fill out the application (application below) and email with supporting documents, including Site Eligibility Report, Phase I and II Environmental Site Assessments and initial payment receipt to brees@utah.gov. Please include the Applicant’s name and a brief description of the site in the email transmission
  2. Pay the fee online using the link below.

Grama Requests and Oversight Cost

GRAMA Request

This is for prearranged payment resulting from a GRAMA request. Please include your name and the Facility ID# or site name for the documents you are paying for.

Oversight Costs Fee

To reimburse the State for time spent overseeing work for programs such as Voluntary Cleanup Program, Enforceable Written Assurance, and Leaking Underground and Aboveground Petroleum Storage Tanks Cleanup. In the Additional Information box include the site name and invoice number you are paying.
