Does your facility wish to register radiation-generating equipment at a new or existing facility, or are you changing the address on a registration certificate?
YES: Download the X-ray Machine Registration Application Form (DRC-10) (102 KB).
Does your facility wish to add to, or remove radiation-generating equipment from, an existing registration certificate?
YES: Download the New, Transfer and Dispose of X-ray Equipment Form (68 KB).
Does your facility wish to make payment for an annual registration or inspection fee?
YES: Contact the Division of Waste Management and Radiation Control at (801) 536-0200.
