Wastewater Treatment and Disposal Systems:
Operating Permits

Legislation has been passed that gives the Utah Water Quality Board authority to issue operating permits for:

Large Underground Wastewater Disposal Systems (LUWDS)

  • Annual Report Form
    Reporting period: July 1 to June 30. After system is inspected by certified individual, owner mails in report by August 1, 2012 and repeats for each year thereafter—or according to the permit requirements. Mailed to owner when an Operating Permit is issued. It details who is to fill it out, deadline for submittal, and where to return the completed form.
  • First Letter
    Mailed to all Large Underground Wastewater Systems advising them of their responsibility and need to apply for an Operating Permit no later than January 1, 2010.
  • Large Underground Wastewater Disposal Systems: List
    Lists all known Large Underground Wastewater Disposal Systems in Utah by county and district, including available current permits.
  • Large Underground Wastewater Disposal Systems: White Paper
    Describes the purpose of the Operating Permit program.
  • Maintenance Specialists List
    List of individuals who have Onsite Professional Certification Level 2 and/or Level 3 who have requested that their name be put on this list.

  • Operating Permit Delegation
    All counties and health districts have been urged to administer the Operating Permit Program to provide more local control. Many have expressed interest,
    but so far only Weber-Morgan Health Department has formally applied and been granted this responsibility.

    Complete this form and submit to the Division prior to the expiration of your current operating permit.

Subsurface Disposal or Rapid Infiltration Basins (RIB)

Wastewater Land Disposal

These wastewater treatment facilities do not fall under the jurisdiction of a general permit and have individual permits issued.

Individual Operating Permits for Wastewater Disposal

These wastewater treatment facilities do not fall under the jurisdiction of a general permit and have individual permits issued.
